Wpływ hemodializy na ciśnienie wewnątrzgałkowe i ciśnienie perfuzji oka: przegląd interdyscyplinarny Artykuł przeglądowy

Abstrakt
Hemodializa jest najszerzej stosowaną metodą leczenia nerkozastępczego i może powodować zmiany ciśnienia wewnątrzgałkowego. Klinicznie istotny wzrost ciśnienia wewnątrzgałkowego związany z hemodializą opisywano przede wszystkim u pacjentów z upośledzonym odpływem cieczy wodnistej, w tym w oczach z wąskim lub zamkniętym kątem przesączania, w przebiegujaskry pierwotnej lub wtórnej (zwłaszcza neowaskularnej i pseudoeksfoliacyjnej). Za jeden z głównych mechanizmów podwyższenia ciśnienia wewnątrzgałkowego uważa się niezsynchronizowany spadek osmolalności krwi i płynów wewnątrzgałkowych, prowadzący do powstania przejściowego gradientu osmotycznego i przemieszczania się płynu do wnętrza gałki ocznej. Równocześnie hemodializa może wpływać na ciśnienie perfuzji oka, a jego obniżenie zwiększa podatność nerwu wzrokowego na uszkodzenie nawet przy prawidłowych wartościach ciśnienia wewnątrzgałkowego. Wahania obu parametrów są klinicznie istotnym, niedostatecznie rozpoznanym zagrożeniem dla pacjentów z grupy ryzyka. Przedstawione w pracy dane podkreślają znaczenie interdyscyplinarnej współpracy nefrologa i okulisty, a istniejące luki w wiedzy uzasadniają dalsze badania w tym kierunku.
