Abstrakt
Eplerenon, selektywny antagonista receptora mineralokortykoidowego, odgrywa istotną rolę w leczeniu pacjentów z niewydolnością serca ze zredukowaną frakcją wyrzutową. Jego zastosowanie zmniejsza ryzyko hospitalizacji oraz śmiertelność w tej grupie chorych. W związku z tym, zgodnie z wytycznymi Europejskiego Towarzystwa Kardiologicznego, stosowanie antagonistów receptora mineralokortykoidowego ma zalecenie klasy I u pacjentów z niewydolnością serca ze zredukowaną frakcją wyrzutową, a także u chorych po niedawnym zawale serca z frakcją wyrzutową lewej komory ≤40% i współistniejącą niewydolnością serca lub cukrzycą. Jedną z istotnych zalet eplerenonu jest działanie antyfibrotyczne, co może przyczyniać się do zmniejszenia ryzyka arytmii, w tym migotania przedsionków. Ponadto dzięki większej selektywności wobec receptorów mineralokortykoidowych jego stosowanie wiąże się z niższym ryzykiem działań niepożądanych w porównaniu ze spironolaktonem.
