Abstrakt

Nocna napadowa hemoglobinuria to bardzo rzadka choroba związana z nabytą somatyczną mutacją w obrębie genu PIGA komórki macierzystej szpiku. Mutacja ta powoduje brak ochronnych białek na powierzchni leukocytów, erytrocytów oraz płytek krwi. Nadmierne działanie układu dopełniacza na erytrocyty pozbawione białek błonowych, tzw. GPI(-), prowadzi do ciągłej hemolizy z towarzyszącymi licznymi nietypowymi objawami. Jest to najcięższa trombofilia nabyta, a epizody zatorowo-zakrzepowe są najczęstszą przyczyną zgonów u pacjentów z nocną napadową hemoglobinurią. Mogą oni cierpieć z powodu objawów sercowo-naczyniowych, żołądkowo-jelitowych, neurologicznych lub hematologicznych. Bardzo często kierowani są do kilku specjalistów. Celem niniejszego artykułu jest przegląd diagnostyki i postępowania u pacjentów z nocną napadową hemoglobinurią, ze szczególnym uwzględnieniem identyfikacji grup wysokiego ryzyka.
