Abstract

This article discusses the importance of multimodal pharmacotherapy in pain management, with particular emphasis on the combination of paracetamol and tramadol, which relies on the synergistic action of distinct analgesic mechanisms. The mechanisms of action of both drugs, their pharmacokinetic profile, safety, and potential interactions are presented, emphasizing the potential for effective analgesia at lower doses and with a lower risk of adverse effects than with monotherapy. This case report of a 72-year-old female patient with chronic pain due to osteoarthritis and osteoporosis illustrates the complexity of pain management in a patient with multimorbidity and functional limitations. The use of tramadol + paracetamol combination therapy in a multimodal model, along with interventional methods and rehabilitation, has limited efficacy in addressing sleep disorders and quality with adequate treatment. The discussion highlights the general spectrum of indications for this combination, including receptor-mediated, neuropathic, and mixed problems, as well as the principles of individualization and dose titration. The article emphasizes that exercise therapy requires a comprehensive, integrated, and regularly monitored registered physician.
