Abstrakt

Zarówno echokardiografia, jak i rezonans magnetyczny serca (CMR) to nieinwazyjne, powszechnie stosowane metody obrazowe, niewymagające użycia promieniowania jonizującego. Zaletą echokardiografii jest mobilność oraz możliwość obrazowania w czasie rzeczywistym. Zastosowanie CMR natomiast wiąże się z mniejszą liczbą ograniczeń związanych z budową ciała pacjenta. Obie metody pozwalają ocenić anatomię układu sercowo-naczyniowego, strukturę i funkcję mięśnia sercowego. Zastosowanie CMR umożliwia też przedstawienie orientacji tętnic wieńcowych.


U pacjentów z ostrym bólem w klatce piersiowej wykonanie badania echokardiograficznego pozwala szybko potwierdzić zawał mięśnia sercowego. Echokardiografia jest metodą z wyboru pozwalającą uznać rozpoznanie większości kardiomiopatii za pewne. Jednakże, w przypadku arytmogennej kardiomiopatii prawej komory, z powodu braku jednoznacznych echokardiograficznych kryteriów, większe znaczenie diagnostyczne ma wykonanie badania CMR. Również w diagnostyce zapalenia mięśnia sercowego bardziej przydatny jest CMR. Rezonans magnetyczny uważa się za złoty standard w obrazowaniu patologicznych mas w obrębie jam serca. Badanie echokardiograficzne odgrywa dużą rolę w stawianiu diagnozy i ocenie stopnia zaawansowania wad zastawkowych, chorób wrodzonych serca i w ocenie większości chorób osierdzia.


W większości przypadków, dzięki mobilności i szerokiej dostępności, echokardiografia stanowi metodę z wyboru przy potwierdzaniu diagnozy. Jednakże, w niektórych sytuacjach, zastosowanie CMR jest bardziej wartościowe. Postaraliśmy się przedstawić zalety każdej z metod w określonych sytuacjach klinicznych.
