Abstrakt

Migotanie przedsionków (AF, atrial fibrillation) jest jedną z najczęściej występujących arytmii. Jego leczenie polega na terapii antyarytmicznej i dążeniu do utrzymania rytmu zatokowego lub kontroli częstotliwości rytmu serca oraz zapobieganiu zdarzeniom zakrzepowo-zatorowym. Od wielu lat prowadzone są badania nad wprowadzeniem nowych, skutecznych i bezpiecznych leków antyarytmicznych, dlatego duże nadzieje wiązano z dronedaronem. Na podstawie wyników badań DAFNE, EURIDIS, ADONIS i DIONYSOS udowodniono, że dronedaron jest skuteczniejszy w utrzymywaniu rytmu zatokowego niż placebo, a mniej skuteczny niż amiodaron. Jednocześnie zaobserwowano, że długotrwałe stosowanie dronedaronu wiąże się z mniejszą liczbą działań niepożądanych. Przedwcześnie zakończone badania ANDROMEDA i PALLAS ujawniły ograniczenia stosowania dronedaronu. Na podstawie aktualnych wytycznych dotyczących postępowania u chorych z AF nie zaleca się stosowania dronedaronu w żadnej postaci niewydolności serca ani nawet w bezobjawowej dysfunkcji skurczowej lewej komory. W chorobie niedokrwiennej serca dronedaron stał się lekiem drugiego rzutu, ustępując pierwszeństwa sotalolowi. Utrzymano możliwość zastosowania dronedaronu w nieistotnej klinicznie chorobie strukturalnej serca i w nadciśnieniu tętniczym powikłanym przerostem mięśnia sercowego. Badania dotyczące dronedaronu pokazały, że lek ten jest słabym konkurentem innych, starszych leków antyarytmicznych.
