Abstrakt

Przewlekła profilaktyka choroby zakrzepowo-zatorowej i terapia doustnymi lekami przeciwkrzepliwymi to jedna z najbardziej powszechnych sytuacji klinicznych, z jaką obecnie spotykają się lekarze różnych specjalności. Dotychczas lekami przeciwkrzepliwymi pierwszego wyboru w wielu wskazaniach byli antagoniści witaminy K, których skuteczność jest ograniczona ze względu na liczne interakcje z innymi lekami, trudno przewidywalny efekt antykoagulacyjny oraz wąskie okno terapeutyczne, co się wiąże z istotnym ryzykiem powikłań krwotocznych lub zatorowych. W ostatnim czasie wprowadzono na rynek leki z grupy nowych doustnych antykoagulantów (NOAC, novel oral anticoagulants), które bezpośrednio hamują czynniki kaskady krzepnięcia, przez co skutecznie zmniejszają ryzyko powikłań zakrzepowo-zatorowych i jednocześnie są pozbawione wielu działań niepożądanych typowych dla innych leków przeciwkrzepliwych. Ponadto ich stosowanie nie wymaga laboratoryjnego monitorowania parametrów krzepnięcia ani dostosowania dawki do masy ciała, a także cechuje je wysoka biodostępność po podaniu doustnym. Do tej grupy leków należy rywaroksaban – wysoce selektywny inhibitor czynnika Xa, skuteczny w zapobieganiu przypadkom żylnej choroby zakrzepowo-zatorowej w wielu sytuacjach klinicznych. Pojawienie się leków z grupy NOAC zrewolucjonizowało codzienną praktykę kliniczną, ale stosując je, należy pamiętać o regułach właściwego prowadzenia terapii, które są inne niż w przypadku antagonistów witaminy K. Artykuł ma na celu omówienie zasad leczenia tymi lekami w najbardziej prawdopodobnych sytuacjach, z jakimi może się zetknąć lekarz praktyk.
